예방접종이 기대한 효과를 가져오려면 접종대상자가 적기에 적절한 방법으로 필요한 접종횟수만큼 접종을 하여야 한다. 이를 위해 연속적이고 지속 가능한 아동의 예방접종기록 관리 방안의 마련이 필요한데, 정보화기술을 이용하여 전국민 예방접종등록사업(national immunization registry)을 도입하였다. 또한 정기 예방접종의 60% 이상을 차지하는 민간의료기관의 참여를 통하여 모든 접종 기록이 하나로 모아져야 가능하다. 따라서 질병관리본부는 민간의료기관의 국가 예방접종사업 참여를 보장함으로써 예방접종률, 자료등록관리, 서비스의 질 향상이 기대되는지를 파악하고자 군포시를 대상으로 시범사업을 수행하였다.
시범사업 초기에는 의료기관의 약품부족 등으로 보건소 접종이 예측치 대비 30.7%나 중가한 것으로 나타나 주민 불편이 야기되었으므로 전국 확대 시에는 충분한 약품공급대책이 동시에 마련되어야 할 것이다. 또한 보건소의 예방접종 분담비율은 39.3%에서 11.9%로 감소하였으며 의료기관의 경우는 60.7%에서 88.1%로 급격히 증가하였다.
뿐만 아니라 민간의료기관들의 민원발생 현황을 보면 대부분 사업 시작 1~2개월에 집중되어 있어 전국 확대 시에는 초기의 민원을 집중 관리할 수 있는 보완책이 요구되었다.
예방접종을 받기 위하여 보건의료기관에서 대기하는 전체 시간은 보건소가 1분 28초, 민간의료기관이 10분 56초로 앞으로 전국 확대 시에는 의료기관에서의 예방접종을 위한 대기시간을 줄이기 위한 노력이 필요할 것으로 보인다. 등록된 자료의 정확성과 함께 비용상환을 위한 적합성을 평가하는데 걸린 시간은 건당 22.4초로 나타났다.
DTaP 1차를 이용하여 예방접종률을 계산해 본 결과 97.6%로 나타났으며, 나머지 2.4%의 경우는 연락이 불가능하여 파악이 안 된 경우여서 예방접종률은 매우 높은 것으로 예측되었다.
피접종자 1인당 평균 보건의료기관이용수는 1.08개소였으며, 전체의 7.5%가 시범사업 5개월 동안 1개 이상 제공기관을 바꾼 것으로 나타났다. 당일 등록률도 보건소 99.1%, 의료기관 94.3%의 결과를 보여 매우 양호하였다.

Objectives: This study was designed to evaluate the feasibility for expansion of national immunization program coverage in Gunpho city, Korea. We expected this project to improve registration rate of private medical facilities, immunization coverage rate, quality of vaccination.
Methods: The period of this project was from Jul 1, 2005 to Dec 31, 2005. And demonstration project area was Gunpho city in Gyeonggi-Do, the number of target population (0~12 years old) was 54,535 persons. Among the 134 private medical facility including 1 general hospital, 7 hospitals and 126 clinics, 64 (47.8%) was participated in demonstration project. Data for analysis utilized main dataset of server computer which registered computerized immunization registry system and documents of project management team. The main focus for analysis was immunization rate, registration of participated medical institutions, total number of institution for immunization per 1 target person.
Results: The immunization rate of 1st vaccination of DTaP was 97.6%, a share of immunization of health center decreased from 39.3% to 10.2%, but that of private medical institution increased rapidly. The waiting time for vaccination was 1 minute 28 seconds in health center, 10 minute 56 seconds in private institution. The time of judging for cost reimbursement by computerized aided system was 22.4 seconds and the rate of this day registration from vaccination to registration on the national immunization registry system was 99.1% in health center, 94.3% in private institution. The average number of institutions per 1 target person for immunization was 1.08, 7.5% of target person changed immunization institution during 5 months.
Conclusions: The demonstration project in Gunpho city revealed positive results in the aspect of vaccination rate, registration rate and quality, consumer’s accessibility, quality of vaccination service. Therefore, the more aggressive participation of private medical institution is expected to improve the expansion of national immunization program coverage.
